Position Summary

Are you a Registered Nurse who thrives on working independently, but also enjoys being part of an interdisciplinary team? Then we have the opportunity for you within our Community Care team in Cranbrook!

Our outstanding Home Health Nurses work collaboratively in the community with clients, family and other care providers to deliver nursing services and support the unique needs of clients with acute, chronic, palliative or rehabilitative needs.

The Home Health Nurse supports the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. In collaboration with the client, family and other care providers the Home Health develops, implements and adapts a care plan, delivers direct care, assesses service eligibility and provides care management to help clients manage their own care and navigate through the various services available within the community setting. Works in consultation with other system partners and bases practice on nursing science and related content from other sciences and humanities.

Successful Applicants will be trained in Case Management

This is a Casual opportunity. Casual means there are no guaranteed hours and work requirements may vary from 0 – 37.5 hours per week. Casual employment includes: short-notice, on-call availability; workload coverage during a temporary absence; and/or providing relief coverage in a specific position. Once hired into a casual role, employees are eligible to apply for internal postings including permanent part-time or full time positions. Qualifications

Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Candidates must also hold a valid B.C. driver’s license.

Previous home health experience is preferred along with an interest in working with geriatric clients.
